Impact

Institute of Healthcare Improvement (IHI) Triple Aim:

The integrated Physician Network (iPN) practices look to improve the health of the population, enhance the patient experience of care (including quality, access and reliability) and reduce, or at least control, the per capital cost of care.  

The iPN Quality Improvement Program identifies strategies and goals to establish priorities and facilitate program implementation and evaluation. These strategies and goals are taken from the Wagner Chronic Care Management Model (CCM), NCQA Patient Centered Medical Home (PCMH) and the Dartmouth work on Micro systems teamwork models. The goals and objectives are used to direct the quality work of the iPN and are evaluated annually.  The studies and projects are identified in the work plan which is monitored ongoing throughout the year by the Clinical Quality Committee and are the core of the iPN quality improvement efforts.  Use the above sub-menu links to find out more about iPN Diabetes, Prevention, and Patient Satisfaction outcomes.
