Bridges to Excellence / Prometheus Bridges to Excellence is a not-for-profit organization developed by employers, physicians, health care services, researchers, and other industry experts with a mission to create significant leaps in the quality of care by recognizing and rewarding health care providers who demonstrate that they have implemented comprehensive solutions in the management of patients and deliver safe, timely, effective, efficient, equitable and patient-centered care. PROMETHEUS Payment® is a not for profit corporation which has been created to steward the further development and implementation of the PROMETHEUS Payment® model. PROMETHEUS Payment® is specifically designed to: (1) improve quality (2) lower administrative burden (3) enhance transparency and (4) support a patient-centric and consumer driven environment, all while facilitating better clinical coordination throughout health care. Centura Health is Colorado’s largest family of hospitals and health care services and one of the state’s largest private employers, operating 12 hospitals, seven senior living communities and home care and hospice services. Home Health Services across Colorado: Centura Health at Home offers home care, hospice, senior living communities, oxygen, home medical equipment and more. As a leading provider of health care services at home, we continue to see the value in improving patient access to care and quality of care through technology. Centura Health at Home is the first in Colorado Associated Community Health Information Exchange (CACHIE) The Colorado Associated Community Health Information Exchange (CACHIE) is a collaborative project of Colorado Community Health Network (CCHN) and Colorado Community Managed Care Network (CCMCN). With funding from the Health Resources and Services Administration (HRSA), the Agency for Healthcare Research and Quality (AHRQ), The Colorado Health Foundation (TCHF) and member investment dues, Colorado community health centers (CHCs) are joining forces to create a shared information technology system to support quality reporting, analysis, and improvement. Colorado Foundation for Medical Care (CFMC) The Colorado Foundation for Medical Care is Colorado's health care quality improvement organization. CFMC works collaboratively with government programs, health providers, and managed care companies to improve the quality of health care. The Colorado Health Foundation works to make Colorado the healthiest state in the nation by increasing the number of Coloradans with health insurance, ensuring they have access to quality, coordinated care and encouraging healthy living. The Foundation invests in the community through grants and initiatives to health-related nonprofits that focus on these goals, as well as by operating medical education programs to increase the health care workforce. Colorado Regional Health Information Organization (CORHIO) CORHIO is a non-profit organization that facilitates health information exchange to improve care for all Coloradans. Created in 2007, CORHIO serves everyone including consumers, employers, doctors, hospitals, nursing homes, pharmacies, home health agencies, health plans and local health information exchanges that are interested in improving the health of all Coloradans through the use of health information technology. CORHIO provides collaboration/convening services to communities and offers secure and confidential technical services where needed. Growing Home began as the Adams County Interfaith Hospitality Network in 1998, a community based shelter for families with children, relying on partnerships with 30 local congregations, a hospital and a local university to shelter families. As the cost of living rose and family homelessness and hunger grew, Growing Home focused on responding to the needs of struggling families in the community. Over the past six years, Growing Home has added homeless prevention services, children’s programming, parenting support, a food bank and affordable housing. In 2007 we reached more than 4,000 households, over 12,000 individuals. Health TeamWorks is a non-profit multi-stakeholder collaborative working to redesign the healthcare delivery system and promote integrated communities of care, using evidence based medicine and innovative systems. Our goals are to optimize health, improve quality and safety, reduce costs, and improve the care experience for patients and their healthcare teams. MobileMD’s 4D HIETM provides secure clinical and administrative data exchange between all providers caring for patients within a community, regardless of their location, EMR technology or vendor. MobileMD’s web-enabled Enterprise Access application further provides support for physician practices that have yet to implement an EMR. In short, MobileMD provides 100% of the physician community with real-time, secure clinical and administrative data exchange, sharing, ad hoc communications and disparate system integration, offering the best path to achieving "meaningful use". |
